Support to Help You Stay Independent

The Community Living Program helps older adults remain safe, supported, and independent at home. These services are funded by state and federal dollars and are available to eligible residents in Livingston, Macomb, Monroe, Oakland, St. Clair, and Washtenaw counties. There is no cost to participate. All services are fully covered, and while voluntary contributions are welcome, they are never required and do not affect your ability to receive care. We work with you—and anyone involved in your care—to understand your needs and develop a plan that fits your situation.


Services May Include:

In-Home Support

Support with daily tasks like medication reminders to help you remain independent at home.

Family Respite

Short-term care that gives family caregivers time to rest while ensuring their loved one continues to receive support.


Assistive Devices and Technology

Support identifying tools or devices that may help you remain safer and more independent at home.

Light Housekeeping

Help with routine household tasks to support a safer living environment.

Meal Preparation

Assistance with preparing simple meals at home.

Shopping and Errands

Support with essential errands, such as grocery shopping or picking up needed items.

Transportation

Help connecting to transportation options for medical appointments, errands, or other essential needs.

Help Finding Other Resources

No matter which services you receive, we’ll help you explore additional community programs or supports that could benefit you.

Program Availability and Wait Times

Community Living Program services are based on eligibility, need, available funding, and program fit. Some services may have wait lists, and availability can vary by county and provider.
